The ultimate goal of Endodontics, both Surgical and Non-Surgical, is to treat or to prevent endodontic pathology by properly cleaning, disinfection and obturation of the complete root canal system. Challenging endododontic retreatments once considered questionable have became successful and predictible procedures with the advancements of modern technologies and the achievement of new scientific acquisitions. Furthermore the periapical lesions persistent after an orthograde approach can now be completely solved by a microsurgical approach.

Based on clinical evidence and supported by current literature, this presentation will illustrate step by step a huge variety of clinical scenario where endodontic retreatmets or microsurgery are indicated. Clinical strategies, specific techniques, appropriate instruments and materials will be in detail discussed and analysed.
